FIRST VISIT

COMPLETION OF NEW PATIENT FORMS

YOUR INITIAL APPOINTMENT WILL CONSIST OF :

  • A comprehensive exam

  • A dental cleaning

  • Digital X-Rays

  • A consultation and review of your treatment options with our dentist

WHAT TO BRING TO YOUR INITIAL CONSULTATION:

  • Any X-rays that were taken by a previous dentist, if applicable.

  • A list of medications you are presently taking.

  • If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-RAYS

  • If a previous dentist has taken x-rays, you may request a copy or they can be faxed to our office at 931-520-8899.

FINANCIAL POLICY

PATIENT BILLING

For your convenience, we accept cash, checks and credit cards (Visa, MasterCard, American Express and Discover). We also offer financing through Care Credit. We deliver the finest care at the most reasonable cost to our patients. Therefore, payment is due at the time service is rendered unless other arrangements have been made in advance. 

 INSURANCE

We are in-network with numerous insurance companies. Please contact the office to verify acceptance of your plan. Insurance plans are an agreement between the patient and the insurance company. Patients should be aware of their insurance co-payments and limitations. As a courtesy, we will gladly file insurance claims on your behalf.  The patient’s estimated portion is due at the time of service. Please notify the office with any changes or updates to your insurance.
